  3. O'Malley defeated Barry in a 2 Out Of 3 Falls Match. On April 16, 2005 NWA Oklahoma was renamed NWA Universal.
  4. O'Malley vacated the title when he departed the company due to "creative differences". Shady won the title by winning the Sooner Stampede on April 14, 2007 and was the last NWA-U Oklahoma Heavyweight Champion. He was recognized as NWA-OK Oklahoma Heavyweight Champion when the company reopened on May 31, 2008 back under the NWA Oklahoma name.
  5. Albright def. Shady to unify the NWA-OK Oklahoma Title and the NWA-OK Universal Title
  6. Albright relenquished the title on September 5, 2008 due to the overwhelming schedule he faced as NWA World Champion. Spoiler 2000 def. Slam Shady for the vacant title.
  7. Griffin def. Carnage for the title in a Triple Threat Match that also included Chad Thomas. This match took place at an NWA Southwest/Panhandle Wrestling Federation event in Amarillo, TX
  8. Faith def. Griffin for the title by winning a 4-Corner Elimination Match that also included Brad Michaels and Kevin Douglas.
  9. NWA-OK ran its last event on June 11, 2011

Albright unified this title with the NWA-OK Oklahoma Title on June 13, 2008. The title was then discontinued.
